Types of Diabetes Management

Patients begin the program with a referral from the physician who is managing their diabetes. It could be your primary care physician, endocrinologist or OBGYN. Then, our insurance team will inform you about your available coverage. You will then complete an intake packet with questions pertaining to your diagnosis, and your first visit will be with a certified diabetes educator for an assessment.

After you complete the assessment, the diabetes educator will recommend group or individual work. Group will include four education sessions once a week for two hours to discuss the seven behavior changes to improve health. Individual work will focus on the same behavior changes. Upon completion of the group or individual work, then you will complete both three and six month post-education assessments. These assessments will serve as a refresher to the principles and to assess lifestyle changes.
